Chronic Disease Management Program Do you have a chronic health condition like diabetes, high blood pressure, or asthma? If so, you are not alone! Chronic conditions become even more common as people get older and often require major changes in lifestyle to cope with their effects. You can learn ways to better manage your health and enjoy a full life.
At the Edmonton West Primary Care Network, we offer a new program to help our patients who are living with chronic health concerns. We have a team of health experts to support you, along with doctors, specially trained nurses,dieticians and pharmacists. This team works with you right in your doctor's office!
Not only will you have routine visits with your doctor, but you may also have visits with other members of the expert team, like the
nurse or dietician. Sometimes you might even see the doctor and nurse together. There may be a chance to meet in a group setting with other patients to learn more about your health condition and ways to better manage the impact this has on your life.
We offer this program to patients of doctors who are part of the Edmonton West Primary Care Network. Ask your doctor or office staff about getting more support in coping with your chronic health condition. During an office visit, your doctor might suggest that you see another member of the team of health experts. The doctor might also offer these services by sending you a letter or through a phone call.

HoursThe After-hours Clinic is for minor injuries and illnesses that are not life threatening, including those that would be referred to a family physician during regular office hours. For more serious ailments, proceed directly to your nearest hospital Emergency Room. Low Risk Maternity Clinic
9:00 AM-12:00 PM - Monday to Friday- for patient appointments The office is open for booking appointments by phone until 4:30 PM Closed stats and week-ends Located in the Meadowlark Professional Centre # 309 8708-155 St Edmonton, AB Phone: (780) 489-6008 Fax: (780) 489 6028The Edmonton West Primary Care Maternity Clinic offers obstetric patients excellent care during their pregnancies. The goal is to provide a service to patients and their existing family physicians for the pregnancy and delivery, including newborn hospital care. The physicians in the maternity clinic will work with a specialist if complications arise during the pregnancy, labor or delivery. Mothers and their newborns will resume care with their own family physician following delivery. All patients will need
to have a family physician; the clinic will give you a list of physicians accepting new patients, but it is your responsibility to call and make the appointment. You are required to advise the clinic when you have found a family physician. You should start looking for a family physician as soon as you are aware you are pregnant. Participating MDs: Dr. Anne Bowland, Dr. Dianne Brox, Dr. Wendy Dawson, Dr Nancy McNeilly, Dr. Jeff Shmoorkoff, and Dr. Cynthia Soneff. Referrals can be made at any time, once pregnancy is diagnosed, by calling the clinic's office during operational hours. We will take referrals at any gestational age of the patient, depending on that physician's expertise/desire to continue Prenatal Care to a specific point. It is hoped that referrals will be made no later than 32 weeks, so the patient has time to meet all of the participating delivery physicians. All deliveries will be done at the Misericordia Hospital. A physician is on call 24 hours a day to deal with antenatal and delivery problems.
